What Is Fluoride and How Does It Work?
Fluoride is a naturally occurring mineral found in soil, water, and many foods. In dentistry, we use it specifically because of how it interacts with tooth enamel.
When bacteria in the mouth consume sugar, they produce acid that dissolves the mineral structure of teeth a process called demineralisation. Fluoride works in two ways:
- Remineralisation fluoride is incorporated into tooth enamel during the repair process, creating fluorapatite, which is significantly harder and more acid-resistant than natural enamel
- Antibacterial at higher concentrations, fluoride directly inhibits the bacteria responsible for cavities
The result: teeth that are more resistant to decay. Multiple decades of large-scale studies consistently show that fluoride reduces childhood cavities by 20–40%.
The World Health Organization, American Academy of Pediatric Dentistry, Indian Dental Association, and virtually every major dental and public health body globally endorse appropriate fluoride use for children. The evidence base is robust and spans over 70 years.
The Concerns Parents Have Addressed Honestly
Dental Fluorosis
Dental fluorosis is the most common concern and the one that's real but usually overstated. It occurs when children swallow too much fluoride during tooth development (typically before age 8, while enamel is still forming).
Mild fluorosis causes faint white lines or spots on teeth mostly cosmetic and often unnoticeable. Severe fluorosis (brown staining, pitting) requires much higher chronic exposure than normal toothpaste use produces.
The key: dosage. The amounts we recommend are carefully calibrated to prevent fluorosis while maintaining the cavity-prevention benefit.
What About Fluoride in Water?
Many Indian cities have naturally occurring fluoride in groundwater. The WHO's safe upper limit is 1.5 mg/L. In high-fluoride areas, dietary fluoride from water reduces the need for supplemental fluoride. If you're in a high-fluoride area, let us know we factor this into our recommendations.
Swallowing large amounts of fluoride toothpaste regularly. The solution is simple: use the right amount (see below), and teach your child to spit. Children under 3 who can't yet spit should have toothpaste applied by a parent a grain-of-rice sized smear only.

Professional Fluoride Applications: When Are They Recommended?
In-clinic fluoride treatments go beyond what toothpaste can do. We use fluoride varnish a highly concentrated fluoride applied directly to tooth surfaces. It takes about 60 seconds, tastes pleasant, and is completely safe.
Who Benefits Most?
- Children with active cavities or high cavity risk
- Children with deep grooves in their molars
- Children with poor salivary flow (e.g., due to medications)
- Children who don't brush consistently with fluoride toothpaste
- Children with orthodontic appliances (braces create harder-to-clean surfaces)
For most children, we recommend fluoride varnish every 6 months at their routine check-up. For high-risk children, we may recommend every 3 months.
Fluoride-Free? What You Should Know
I understand that some parents prefer fluoride-free toothpastes. I respect this decision and I want you to make it with accurate information. Fluoride-free toothpastes do not prevent cavities in the same way. Some contain hydroxyapatite as an alternative the evidence for this ingredient is growing but not as established as fluoride.
If you choose fluoride-free, your child is at higher cavity risk and will need more frequent dental check-ups, excellent sugar control, and possibly fluoride varnish applications at the clinic (some parents are comfortable with professional fluoride even if they avoid it at home).
