LASER Operculectomy — Removing the Gum Flap Over an Erupting Molar

When a back molar — particularly a second permanent molar — is erupting, a flap of gum tissue can trap food and bacteria, causing pain and infection (pericoronitis). LASER operculectomy removes this flap precisely in a single short appointment, ending the recurring pain without stitches.

What Is an Operculum?

An operculum is the flap of gum tissue that partially covers an erupting molar, typically the lower second permanent molar (erupting around age 11–13). Food and bacteria collect under this flap, causing pericoronitis — pain, swelling, and sometimes spreading infection.

Repeated episodes of pericoronitis are common because the flap remains even after the acute infection clears. LASER operculectomy removes the flap permanently, allowing the tooth to erupt cleanly and ending the cycle of infection.

Why LASER?

The Procedure

The area is numbed with topical gel and a small local injection. The LASER handpiece is then used to remove the opercular flap in 5 to 10 minutes. There is no bleeding, no stitches, and no wound that needs dressing. The child can eat soft foods the same evening.

Rarely, if the excision was not complete. If the molar is still erupting, a small rim of tissue may reappear, but the recurrent infections almost always stop. We advise on the timing of the procedure relative to the tooth's eruption stage to minimise this.

Yes — recurring pericoronitis is the primary indication. Antibiotics treat the acute episode but do not prevent recurrence; removing the flap does. Three or more episodes is a clear indication to proceed.
