The Golden Rule: First Tooth or First Birthday

The American Academy of Pediatric Dentistry (AAPD) and the Indian Society of Pedodontics & Preventive Dentistry both recommend the same thing: your child's first dental visit should happen within 6 months of the first tooth erupting, or by 12 months of age whichever comes first.

In my 18+ years of practice, I've seen the consequences of delayed first visits cavities in 2-year-olds, baby teeth lost too early, severe dental anxiety in older children who were never introduced to dental care gently. Almost all of it was preventable.

Why So Early? Babies Don't Have Many Teeth Yet

This is the most common question I hear. Here's why early visits matter:

  • Baby bottle tooth decay one of the most common early childhood diseases typically starts between 6–18 months. Catching it early makes a huge difference.
  • We assess jaw development, gum health, and eruption patterns that set the stage for permanent teeth.
  • We counsel parents on feeding habits, pacifier use, and thumb sucking before these become problems that require intervention.
  • We apply fluoride varnish to protect new teeth from the very start.
  • Most importantly your child learns that dentists are friendly, which prevents dental anxiety from developing.

Age-by-Age Dental Milestone Guide

0–12 months: Before the First Tooth

Even before teeth arrive, good habits start. Wipe your baby's gums with a damp cloth after feeds. When the first tooth appears (usually around 6 months), switch to a soft infant toothbrush with a grain-of-rice sized smear of fluoride toothpaste.

First dental visit by 12 months. We do a knee-to-knee exam gentle, quick, and designed for infants.

1–3 years: The Toddler Years

Visit every 6 months. We monitor eruption of baby teeth, assess bite development, and apply fluoride varnish. We also address habits like thumb sucking and sippy cup use that can affect tooth and jaw development.

Watch Out For

White spots on teeth (early cavities), brown or black staining, teeth that don't erupt on schedule, or a child who complains of tooth pain these all warrant an immediate visit regardless of age.

4–6 years: The Pre-School Stage

Baby molars are now all erupted. This is prime time for pit and fissure sealants protective coatings applied to the deep grooves of molars where 80% of childhood cavities begin. Quick, painless, and incredibly effective.

We also do a first orthodontic screening at age 6 to identify any early warning signs that benefit from interceptive treatment.

7–12 years: Mixed Dentition Period

Baby teeth are falling out, permanent teeth are coming in. This is the most important period for orthodontic assessment. Crowding, bite issues, and spacing problems are far easier to correct when the jaw is still growing.

Every child should have an orthodontic evaluation by age 7 this doesn't mean they need braces, but it means we catch things early when treatment is simpler and less expensive.

13–18 years: Teenage Dentistry

Wisdom teeth, braces completion, sports guards, and age-appropriate cosmetic treatments like teeth whitening become relevant. Teenagers need to own their oral health we help them develop the self-motivation to maintain excellent habits independently.

How to Prepare Your Child for Their First Visit

  • Use positive language. Avoid words like "hurt," "pain," or "needle." Instead, say "the doctor will count your teeth and make them sparkly clean."
  • Read books about dentist visits "The Berenstain Bears Visit the Dentist" is a classic that works beautifully.
  • Play dentist at home let your child examine your teeth with a flashlight. Role-playing normalises the experience.
  • Don't use dental visits as threats this creates negative associations that can last decades.
  • Bring a comfort item a favourite toy or blanket is always welcome at our clinic.
